Birth, Death, Marriage Certificates
Certified copies from the Register of Vital Records (birth, marriage and death certificates) are issued by the Office of Vital Records in the Municipality where the birth, death or marriage occurred (offices of city districts in Prague). Copies of birth certificates for Czech citizens born abroad can be obtained from the special register maintained by the Office of the Municipal District of Brno-střed.

In the event of death, the relevant registry office must make an entry regarding this registry event in the registry book and issue a death certificate for the deceased.
Criminal Record Certificates
Czech Criminal Record Extracts (Výpis z rejstříku trestů) are available online through the Citizen Portal (with eGovernment mobile key, bank identity, mojeID). Extracts can also be obtained in person from any Czech POINT office (in Czech) (in Czech) or from aboard by applying to your nearest Czech embassy
